Member Application

Upon receipt and review of a fully completed application, the Elections Committee shall deliberate and if appropriate, submit the applicant’s name for approval by a membership vote. Two (2) sponsors are required for WNYEPC applicants.  One of the sponsors must be an active member in the same specialty area, but cannot come from the applicant’s place of employment.  The Bylaws governing the application process may be found under Documents posted on www.wnyepc.com

Following your submission of the  application below, you will need to submit 3 signatures by email to [email protected] and [email protected] :  Two (2) signed sponsorship forms and your signature page with initials where indicted.  The entire application form is attached here
